Transition
Transition planning prepares students with disabilities for life after high school. In California, transition planning must begin by age 14. The IEP must include measurable postsecondary goals and the services needed to achieve them.
1Transition Planning Requirements
The IEP must include transition services beginning at age 16 under federal law and age 14 in California. Transition services must be based on the student's strengths, preferences, interests, and needs, and must include measurable postsecondary goals.
2Vocational Rehabilitation
The California Department of Rehabilitation (DOR) provides vocational rehabilitation services to eligible students with disabilities. Students should apply to DOR by age 16 to ensure services are in place before they exit special education.
3College & Higher Ed (504/ADA)
Students with disabilities who attend college are protected by Section 504 and the Americans with Disabilities Act (ADA). Colleges are not required to provide the same services as K–12 schools, but must provide reasonable accommodations.
4Summary of Performance
When a student exits special education (by graduation or aging out), the school must provide a Summary of Performance (SOP) documenting the student's academic achievement, functional performance, and recommendations for postsecondary goals.
5Future Planning & Adult Services
Adult services may include Regional Center services, supported employment, independent living programs, and more. Planning for adult services should begin early — ideally by age 16 — to avoid gaps in services.

The California Department of Rehabilitation provides vocational rehabilitation, independent living, and other services to individuals with disabilities. Contact your local DOR office to learn about eligibility and available services.
